Position Summary

We are seeking a dedicated Licensed Practical Nurse (LPN / T-LPN) to join our team in Concord, NH for a 2nd shift contract assignment. The LPN will provide high-quality patient care, support daily clinical operations, and ensure a safe, compassionate environment. This role is ideal for a reliable, patient-focused nurse who excels in afternoon and evening care settings.

Key Responsibilities

  Deliver direct patient care under the supervision of an RN or clinical leader

  
  


• 
  

  Monitor patient conditions and report changes promptly

  
  


• 
  

  Administer medications and treatments according to state guidelines

  
  


• 
  

  Assist with ADLs and routine nursing tasks

  
  


• 
  

  Perform wound care, injections, and clinical procedures as assigned

  Document care accurately and in compliance with regulations

  Maintain a clean, safe, and supportive care environment

  Communicate effectively with patients, families, and interdisciplinary teams

  
  

Qualifications

• 
  

  Active New Hampshire LPN license or compact equivalent

  
  


• 
  

  Current BLS/CPR certification

  
  


• 
  

  Strong assessment, communication, and organizational skills

  
  


• 
  

  Experience in long-term care, skilled nursing, or rehabilitation preferred

  Reliable, compassionate, and comfortable working 2nd shift hours
