Licensed Practical Nurse

Overview

Your Opportunity: Reporting to the Area Manager, Red Deer Home Care, you will interact with the team and clients/families to provide quality care to clients in the community. As part of the Intensive Home Care and Community Response team you will work closely with clients/families and members of the health care team to facilitate early hospital discharge and to prevent hospital admissions when appropriate. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates.
