About The Role

The Long-term Care Coordinator practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model. The Long-term Care Coordinator (LTCC) is responsible for the coordination of day-to-day operations and quality individual care within a designated clinical area(s).
