MDS Coordinator

Affinity Healthcare is currently hiring for the position of MDS Coordinator.

Job Summary: the MDS coordinator is responsible for assuring the completion of each resident's MDS and care plan. This includes ongoing assessment of each resident's health care needs and direct communication with care staff to provide and evaluate the effectiveness of the plan of care.

Reports to: Director of Nursing

40 hour per week, hourly position

Responsibilities:

• Tracks and schedules required resident assessments per state and federal requirements

• Completes all Minimum Data Sets (MDS) assessments and Care Area Assessment Summaries (CAAs)

• Transmits all completed MDS assessments to the state in compliance with state and federal regulations and Affinity Healthcare policy

• Completes resident assessments to capture highest possible Patient Driven Payment Model (PDPM) score...
