Just Better Care Macarthur is are looking for an experienced NDIS Support Coordinator, whom is looking to take an exciting role in a home support business that has been providing services for many years and is presently enjoying further growth across Macarthur & Murrumbidgee Lachlan NSW.

Support Coordinators are primarily office based staff who perform an integral role in ensuring services are delivered to National Disability Insurance Scheme (NDIS) customers. They play a capacity building role in enabling participants to implement supports in their plan, including informal, mainstream, community and funded supports. The role involves liaising with service providers and participants to coordinate supports and ensure services are provided in accordance with the participants assessed needs and within the quality and safeguarding standards of the NDIS.

As an experienced NDIS Support Coordinator you will work in a team to assist clients to achieve their goals as set out in their NDIS plan. You will provide support coordination to clients and assist them with their NDIS applications/Plans and work closely with NDIS participants, their families/guardians and ensure they get the most out of their funding and achieve great outcomes.

Duties and responsibilities include:
  • Research, coordinate and manage a range of supports to suit individual needs across multiple providers Coordinate access to support items in an NDIS Participant’s Plan in line with the preferences of the participant to achieve the outcomes identified in the plan
  • Support participants to strengthen their ability to connect with informal, mainstream and funded supports, and to increase capacity to maintain support relationships, and resolve service delivery issues within their local communities
  • Support individuals to build their capacity and networks with the aim of greater choice, control and independence in managing their personal lifestyle choices
  • Strengthen and enhance the participants capacity to coordinate supports, self-direct and manage their own supports and participate in community
  • Ensure all reports are written in accordance with the NDIS framework and in a timely manner
  • Assess mainstream, community and informal provider options and link and guide participants through their choices
  • Negotiate services to be provided and their prices (including quotable services), develop service agreements, and create service bookings
  • Liaise with plan managers regarding funds and claim management categories
  • Ensure that service provision is informed by the participants’ individual needs and is in line with current participant protocols or individual service plans, from JBC or other services engaged through Supports Coordination
  • Work with customers and their natural supports to assess the quality and value of the supports provided and ensure that they are meeting the participant’s individual needs
  • Actively support participants to connect with their communities and encourage participation in both mainstream and community options
  • Affirm and record progress of participants identified goals to be achieved through their supports
  • Where appropriate, assist the participant to get ready for their plan review
  • Where appropriate, assist participants to decide on what actions to take in order to achieve goals in relation to exploring housing options and life transition planning
  • Ensure quality standards are maintained at all times and adhere to Just Better Care’s Quality Management System and the NDIS quality and safeguarding practice standards
  • Respect the privacy and dignity of all participants
SELECTION CRITERIA :
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector
  • Demonstrated high level awareness of issues affecting the disability sector and people with disabilities
  • Experience in providing services to customers including assessment and monitoring processes and other services including mainstream
  • Experience in (or the willingness to learn) risk assessments within a workplace safety context
  • Demonstrated experience in working toward targets to meet program outputs and outcomes
  • Demonstrated high level verbal and written communication skills, including familiarity with and confidence in using, the Microsoft Suite
  • Demonstrated commitment to working within a strengths based, person-centred framework
  • Sound leadership skills
  • Proven relationship management with all internal and external stakeholders, as well as good interpersonal and negotiation skills
  • Current and clear Criminal Record Check and Working with Children Check
  • Ability to work effectively, efficiently and accurately and prioritise workloads
  • Demonstrated problem solving skills and the ability to leverage networks
  • Relevant tertiary qualification
  • Experience and skills to work competently alongside people, their family and carers in their local community to build their capacity to participate in the community and manage some or all aspects of their plan.
  • Previous management or coordination experience in the disability, health/aged care, not for profit, or community services sector
What you will need:

NDIS Worker Screening Clearance / Criminal Record Check

  • Customer service skills demonstrated by experience

Qualifications in Social Work, Allied Health, Case Management or other relevant areas.

Knowledge/experience in NDIS Support Coordination and or NDIS Audit requirements

Previous experience in the community care or health/aged care sector

Drivers Licence

  • First Aid & CPR (preferred)
  • COVID-19 Vaccinations up to date

This is a full time role and is operated in Macarthur/Wagga Wagga region working from Gledswood Location.
