Job Description

Schedule: Mon - Fri 8am-4pm / Flexible schedule within business hours / weekends off! 

Salary: $85,000 / year 

Sign-on Bonus: $4,000; $2000 upon 30 days, $2000 upon completion of 6 months

As a Nurse Liaison, you will function as a nurse educator and consultant in collaboration with other clinicians regarding health promotion and maintenance, medication administration, and acute and chronic medical / psychiatric crisis management for persons served. Nurse liasons will provide support and carry a case load of individuals living within DDS group homes/supported appartments. This includes attending team meetings, monthly reviews, rounding at the various programs, training of employees in various nursing tasks/delegations and participating in a team process of referral reviewed/admissions.
