Job Type: Full-Time

Schedule: 7 days on / 7 days off (rotating)

We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality healthcare services in a Skilled Nursing Facility (SNF) in Lynchurg, VA.

 

The Nurse Practitioner (NP) will work closely with physicians, nursing staff, and other healthcare professionals to assess, diagnose, and treat patients while ensuring continuity of care. This role is ideal for a Nurse Practitioner (NP) who is passionate about geriatric care and committed to improving patient outcomes in a long-term care setting.

Duties and Responsibilities

  • Conduct comprehensive patient assessments and develop individualized treatment plans

  • Diagnose and manage acute and chronic conditions in coordination with physicians

  • Educate patients, families, and facility staff on disease management and preventive care

  • Maintain accurate and timely documentation in compliance with regulatory guidelines

  • Collaborate with interdisciplinary teams to ensure quality patient outcomes

Skills and Qualifications

  • Ability to work independently while collaborating with a multidisciplinary team

  • Excellent communication and interpersonal skills
