The role focuses on dementia care within our Residential Aged Care Homes and involves leading advanced practice, collaboration with GPs and hospital outreach, and reducing avoidable transfers.

Key Responsibilities

• Deliver comprehensive NP level assessment and management for residents with dementia, including non-pharmacological and pharmacological strategies within scope and legislation.
• Provide onsite clinical support to care teams, collaborate with GPs, and coordinate case conferences to ensure continuity of care across primary care and LHD.
• Build capability through targeted education and coaching for RNs, care staff, and families to strengthen dementia health literacy.
• Contribute to program reporting and outcomes under the CESPHN funding agreement.
