Nurse Practitioner / Physician Assistant – Primary Care Float / Sub-Zero

Work Schedule:

80-100% FTE, Monday – Friday between the hours of 7:00 AM – 5:00 PM. No holidays or call.You will work 50% at a variety of primary care clinics within UW Health including Family Medicine and General Internal Medicine within Madison, WI and the surrounding regional clinics. You will also work 50% at the Sub-Zero Employer On Site Services, located in Fitchburg, WI. During the week you work at the Sub-Zero Clinic – you will have a half day on Friday.

We are seeking a Nurse Practitioner OR Physician Assistant to:

Float to a variety of clinics. You will rotate one week at the UW Health Sub-Zero Employer On Site Services, located in Fitchburg, WI and one week at a variety of clinics within UW Health including Family Medicine and General Internal Medicine within Madison, WI and the surrounding regional clinics.

The Sub-Zero Clinic is a dedicated on-site healthcare facility at the Sub-Zero Group headquarters in Fitchburg that supports employees and their families.

This role provides a unique opportunity to collaborate across various locations and foster professional connections throughout the Madison area. 

Perform annual physical exams and help keep patients up to date on health maintenance.

Care for the acute and chronic needs of patients from newborn to elderly.

While at the Sub-Zero Clinic, you will be the only advanced practice provider (APP) in the clinic, requiring the ability to make decisions independently. 

An APP with at least two years’ experience in Family Medicine, Primary Care or Internal Medicine would succeed in this role. 

Education:
If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.

Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure
If Nurse Practitioner*:

Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND

Active board certification as a NP in area of clinical practice. These may include the following: Family.

If Physician Assistant*:

Active PA license, including prescriptive authority, in state(s) of practice AND

Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire
