Nurse Practitioner

Carecor is seeking compassionate Nurse Practitioners (NPs) to deliver virtual and mobile substance‑use care to individuals experiencing homelessness in Toronto. Using a harm‑reduction and client‑centered approach, NPs provide assessment, treatment, and crisis support for clients with complex substance‑use disorders, including opioid, alcohol, and stimulant use disorders.

Summary of Position

The Substance Use NP works collaboratively with nurses, physicians, shelter case managers, community workers, and agency staff to deliver substance‑use care and limited primary care to clients experiencing homelessness, in alignment with PHIPA and client consent requirements.

The NP provides comprehensive, advanced assessments supported by the mobile substance‑use nurse to address both urgent and ongoing substance‑use needs, escalating concerns to a physician or Emergency Services when appropriate.
