Position Summary
This position with the Providence Care – Transitions Home Program will be engaged in community work and involves commuting across the Southeastern Ontario region. The Transitions Home Program works at achieving timely access to care and clients' functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client's home and community.

The Occupational Therapist, reporting to the Clinical Program Coordinator, acts as the fundamental resource to program staff, clients, caregivers and community partners.
