Occupational Therapist I

Your Opportunity:

This exciting community based Occupational Therapy Interventionist position reports directly to the designated Care Manager within the Calgary Zone Integrated Home Care Program, Adults Team. The Occupational Therapist works within the full scope of their practice as a member of an interdisciplinary health care team. Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach you will develop care plan(s) and provide direct care for client(s)/family(s) by identifying their needs, focusing on strengths, and setting achievable personal health and self-care goals. In collaboration with the client(s)/family(s) and interdisciplinary team you will ensure care and services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service provision.
