Job Summary:

Provides individualized assistance to oncology patients, families, and caregivers to identify and minimize barriers to care. Promotes achievement of organizational and departmental objectives and strategic plan.

Essential Job Duties:

  • Facilitates the patient's timely access to medical and psychosocial care across the continuum from screening, diagnosis, treatment, rehabilitation, survivorship and/or end of life.
  • Coordinates a seamless model of access and care benefiting patients, family members, and participating physicians throughout the GSH system including both primary and secondary service areas.
  • Utilizes a collaborative, interdisciplinary approach to maximize utilization of resources and minimize duplication of services.
  • Maintains effective and timely communication with all members of the patient's healthcare team.
  • Actively seeks and participates in educational opportunities, both formal and informal, to achieve and maintain knowledge and skill related to job responsibilities.
  • Attends patient care conferences, and other cancer program meetings as appropriate.
  • Facilitates interdisciplinary care planning meetings as appropriate.
  • Participates in planning, implementation, and evaluation of community needs assessment at least once every three years.
  • Collects data, documents, and provides report to Cancer Committee on an annual basis to include: identified health disparities, description of navigation process, populations served and barriers identified, documentation of activities and metrics (outcomes), and areas for PI/QI enhancement and future directions.

  • Represent Oncology Program in community activities
  • Assists with marketing strategies to help promote program
  • Breast Navigator will also be responsible for assisting physician in Breast Center as designated by GSPN Operations Manager.
  • Breast Navigator will cross train to provide nursing coverage to the Breast Care Center.

  • Experience: Practical work experience in oncology with preferred experience in outpatient oncology and working with financial aspects of cancer care (CMS, third party payors, .
  • Obtain and maintain applicable certification within one year of becoming eligible.
  • Licensure - Registered Nurse or Registered Radiologic Technologist
  • Certification - Breast Health Clinical Navigator Certification required within 1 year of employment
