About the Role Act as a central point of contact to ensure appropriate support is available. Plan and organise work using own initiative, whilst being able to work as a valuable team member. Be able to make referrals to relevant service providers. Coordinate meetings, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present. Promote and facilitate integration within the GP and community teams.
Mid Norfolk Primary Care Network (PCN) has an opportunity for a care co-ordinator to join their team, supporting six GP practices and linked care homes across the Dereham area, this position can be considered on a full and part time basis.
The role involves working very closely with the GPs and practice teams within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.
