PCN Paramedic Practitioner

We are looking for an experienced Paramedic Practitioner to join our new Enhanced Care & Frailty Team at Dover Town PCN. This is a multi-site role delivering high-quality, person-centred care through home visits and care home support. You will play a key role in proactive care, working closely with our multidisciplinary team to improve outcomes and independence for patients across our PCN area.
