Job Specification:

We're looking for a dedicated Hybrid Pharmacist to join a supportive team in a truly rewarding role that combines aged care and community pharmacy work.

For two days each week , you'll be onsite at a local aged care facility, providing medication reviews, updating histories, and delivering vital advice and education to residents and staff. You'll liaise with the community pharmacy to coordinate medication changes and ensure a smooth flow of service.
