Physical Therapist

Job Description Location: Home health visits throughout Naples Schedule: Full-time, 40 hrs/week (Monday–Friday), with occasional weekend/holiday on-call rotation What You’ll Do Conduct comprehensive patient assessments — including required OASIS evaluations. Develop, initiate, and adjust individualized physical therapy care plans as needs evolve. Provide skilled physical therapy treatments in patients’ homes. Educate patients and families on disease processes, self-care strategies, injury prevention, and home safety. Collaborate with an interdisciplinary home health team to deliver coordinated, patient-centered care. Qualifications Active Florida PT license required. Minimum 1 year of experience as a licensed Physical Therapist. Home health experience preferred (but not required). Comfort with on-call rotations (weekends/holidays) as needed. Strong assessment, critical thinking, and patient education skills. Familiarity with home health EMR systems (e.g. HCHB) is a plus.
