Responsibilities

• Work within the Community Rehabilitation Team and manage a varied caseload of community patients across the north of Angus, including musculoskeletal, neurological, frailty and orthopaedic conditions.
• Collaborate closely with multidisciplinary team members and actively participate in Enhanced Community Support (ECS) meetings across the locality.
• Support the Angus Parkinson's disease multi-disciplinary annual review clinics and deliver the Better Balance programme across Angus.
• Develop and provide the new essential link for the Angus Falls service.
