Team, reporting, and responsibilities for the Primary Care Clinic Nurse Practitioner NP or Physician Assistant PA


The Primary Care Clinic Nurse Practitioner NP will report to the Primary Care Medical Director and Clinic Operations Director while collaborating daily with board-certified Family Medicine and Internal Medicine physicians; the Primary Care Clinic Physician Assistant PA will be supported by friendly nurses, experienced medical assistants, care coordinators, and a diverse team including behavioral health, pharmacy, and social work. The Primary Care Clinic Nurse Practitioner NP or Physician Assistant PA will deliver comprehensive primary care across the lifespan, including preventive services, chronic disease management, acute visits, women’s health, and evidence-based procedures aligned with competencies. The Primary Care Clinic Nurse Practitioner NP or Physician Assistant PA will participate in daily huddles, panel management, quality metrics, and population health initiatives, all enabled by an integrated EHR, e-consults, and streamlined workflows designed for clinician success.
