Mediplacements is pleased to be working with a GP practice / Primary Care Network (PCN) in Hertfordshire to recruit an experienced Primary Care Pharmacist for a 6-month fixed term contract.

This role is ideal for a pharmacist with proven experience in Atrial Fibrillation (AF) reviews, QOF, and Enhanced Care Framework (ECF) delivery, looking to make a real impact within primary care.
The Role
You will support the practice/PCN by delivering high-quality clinical pharmacy services, with a particular focus on AF management and anticoagulation safety. Working closely with GPs and the wider multidisciplinary team, you will help improve patient outcomes while supporting the delivery of key contractual targets.
