Responsibilities

  • Conduct case assessment (MoCA etc), review and formulate Individualised Care Plans for Persons with Dementia.
  • Provide interventions such as psychoeducation, supportive counselling and casework.
  • Provide case recordings and relevant documentation.
  • Plan and conduct Caregiver Support Group sessions.
  • Involved in planning and facilitating individual and group sessions.
  • Assist in the development and introduction of new activities.
  • Assist in procuring materials required for activities.
  • Other duties as assigned.

REQUIREMENTS

  • Degree in Psychology, Social Sciences, Gerontology, or a health-related field, with at least 3 years of relevant experience in a similar role.
  • Able to work independently, has leadership quality and is a responsible team player.
