Responsibilities
- Conduct case assessment (MoCA etc), review and formulate Individualised Care Plans for Persons with Dementia.
- Provide interventions such as psychoeducation, supportive counselling and casework.
- Provide case recordings and relevant documentation.
- Plan and conduct Caregiver Support Group sessions.
- Involved in planning and facilitating individual and group sessions.
- Assist in the development and introduction of new activities.
- Assist in procuring materials required for activities.
- Other duties as assigned.
REQUIREMENTS
- Degree in Psychology, Social Sciences, Gerontology, or a health-related field, with at least 3 years of relevant experience in a similar role.
- Able to work independently, has leadership quality and is a responsible team player.
