Location: Various locations across Dorset available
Responsibilities
Each day as a Reablement Assistant is different. You will support people within their own homes to rebuild their confidence and independence, and assist in achieving their personal goals.
- Provide person-centred care and support in clients’ homes
- Assist with daily living activities and enable independence
- Collaborate with the care team to plan and review support goals
- Ensure safety and well-being of clients at all times
- Maintain accurate records and communicate effectively with clients and families
