Brooks Long Term Care is a 24-bed unit, with 15 long-term care beds and 9 transition beds, located in a rural setting. The Registered Nurse - Long Term Care works as a charge nurse within an interdisciplinary team to provide resident and family-centered care. As a member of the CoAct team, you will be responsible for assessing, designing, and implementing continuing care services for a caseload of clients. You will play a key role in resident care planning (interdisciplinary including resident and family) to help residents achieve their goals and enhance their quality of life. Using principles of patient and family-centered care, you will collaborate with the interdisciplinary team and clients to develop individualized care plans and support clients in meeting their care goals.
