Location: UCHealth UCHlth Anschutz Outpt Pavilion, US:CO:Aurora Department: Hematology Clinic FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks) Shift: Days Pay: $38.91 - $60.31 / hour, based on relevant experience Join our team as a Registered Nurse Care Manager in Hematology! In this role, you will work collaboratively with physicians, interdisciplinary teams, individual clients, and their families to enhance patient outcomes. Your focus will involve assessing, planning, implementing, coordinating, monitoring, and evaluating care needs, using effective communication and educational resources to support quality outcomes at a reasonable cost. Your expertise will help facilitate care across various settings including acute, ambulatory, and specialty care. Key Responsibilities: Utilize critical thinking and the nursing process to meet client healthcare needs based on research and evidence-based practice. Create collaborative partnerships with clients to explore their health needs, decision-making patterns, and resource utilization. Advocate for and educate clients and their families on treatment options, community resources, and psychosocial aspects to foster self-care and independence. Enhance communication and coordination among healthcare team members, engaging clients in the decision-making process to ensure seamless services. To excel in this fast-paced environment, you will need strong critical thinking skills, decisive judgment, and the ability to work independently. Requirements: Graduate from an accredited nursing program; Bachelor's degree in Nursing preferred. Current state licensure as a Registered Nurse (RN); Certified Case Manager (CCM) preferred. Minimum of 2 years nursing experience; Acute care and 1 year of case management experience preferred. BLS certification from the American Heart Association or American Red Cross required prior to start date.
