Your Opportunity: If you are interested in working within an inter-professional team playing a key role in linking the needs of Rural Home Care clients with appropriate health and community services, then this Registered Nurse position will be of interest to you. Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span.
