Job Overview
The RN Interventionist position, with the Adult Team in Home Care, is responsible for assessing, planning, organizing, implementing, reviewing and evaluating the needs of shorter term clients (Acute and Rehabilitation clients) and their caregivers/families requiring community based services. The RN Interventionist fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings. The RN Interventionist is also responsible for providing direct nursing interventions to a select group of Long Term Supportive, Maintenance and End-of-Life clients.
