Job Description

This position providescustomer-based services andinvolvesthe admission and assessment of consumers in line with their needs and goals. By partnering and working with consumers we assist in enabling them to safely live in their own home and community.

The successfulapplicant will be someone who can independently manage and grow a portfolio of Support at Home clients.
