About the Role 

OpsXpress is seeking a high-calibre Senior Backend Engineer to join the core engineering team at  

Emmacare, a leading US-based SaaS platform specializing in Chronic Care Management (CCM) and  

Telehealth. 

In this role, you will be the technical backbone for client onboarding and data integration. You will  

bridge the gap between complex healthcare data ecosystems (EMRs) and our scalable AWS-driven  

platform. This is a long-term career opportunity within a mission-driven US engineering organization. 

Key Responsibilities 

• Architect & Implement: Lead the technical deployment of the Emmacare platform for new US healthcare clients. 

• Data Integration: Build and maintain robust data pipelines using REST APIs, HL7/FHIR, and EDI to ensure seamless exchange with EMRs. 

• Scalability:  Optimize backend services within our AWS infrastructure (ECS, EC2, RDS) to handle high-volume clinical datasets. 

• Technical Leadership: Act as the primary technical point of contact during client onboarding, resolving complex implementation and interface challenges. 

• Documentation: Create rigorous technical documentation for integration workflows and deployment best practices. 

Technical Requirements 

1. PHP Expertise (Strong Knowledge Required) 

 • Advanced PHP: Deep proficiency in modern PHP (7.4/8.x) and extensive experience with the Symfony framework. 

 • Architecture: Proven ability to write clean, testable, and maintainable code following SOLID principles and Design Patterns. 

2. US Healthcare & Data Exchange (Mandatory: 1+ Year) 

 • Industry Experience: Minimum 1 year of experience specifically within the US Healthcare IT sector. 

 • Standards: Practical experience with healthcare data standards, specifically HL7 (v2/v3) and  FHIR. 

• Workflows: Deep understanding of EDI workflows and secure messaging protocols (Direct, SFTP, etc.). 

 • EMR Familiarity: Experience integrating with major US Electronic Medical Record systems (e.g., Epic, Cerner, Athenahealth). 

3. Database Working Knowledge 

• SQL Mastery: Expert-level proficiency in leading databases (MySQL, SQL Server etc.). 

• Performance: Strong experience in query optimization, indexing strategies, and database schema design for high-concurrency environments. 

• Data Integrity: Deep understanding of ACID properties and maintaining data consistency in complex healthcare environments. 

4. Cloud & Infrastructure 

• AWS ecosystem: Daily hands-on experience with ECS, EC2, S3, and CloudWatch. 

• API Design: Expert knowledge of RESTful API design and lifecycle management. 

Soft Skills & Logistics 

• Communication: Fluent English (C1-Advanced) with the ability to lead technical calls with US- based stakeholders and clinicians. 

• Time Zone: Flexibility to maintain significant overlap with US Time Zones (EST/CST) for collaborative engineering sessions. 

• Problem Solving: A self-starter mindset with the ability to debug complex data discrepancies  

independently.
