We focus on integrating health education, enhanced care coordination, and social support through our dedicated team of Community Healthcare Workers (CHW). Our growing team of 100 volunteers helps people most at risk of poor outcomes — including communities of color, rural populations, and people with disabilities — access better care and live healthier lives. The Role We are seeking a highly motivated and experienced Volunteer Senior Program Lead to provide strategic guidance, oversight, and mentorship to our Community Healthcare Worker (CHW) team and to help shape our approach to enhanced care management and community supports. This is a critical, high-impact leadership role ideal for a retired or current healthcare professional looking to give back, or a seasoned program manager passionate about health equity and holistic care models. As a Senior Program Lead, you will play a pivotal role in shaping the future of HHP’s Enhanced Care Management and Community Supports work — helping bridge the gap between community needs and equitable access to care and live healthier lives. Key Responsibilities Strategic Leadership: Work with the Board and Executive Director to refine and scale our enhanced care management and community support initiatives, ensuring alignment with our holistic health literacy mission. Operational Oversight & Coordination: Provide expert guidance and leadership for the operational work of our Community Healthcare Worker (CHW) team. CHW Mentorship & Development: Offer one-on-one and group mentorship to our volunteer CHWs, helping them navigate complex cases, improve documentation, and enhance their care coordination skills. Partnership Identification: Help identify and evaluate potential community-based partners (e.g., social service agencies, food banks, housing programs) to strengthen our network of community supports. Quality Improvement: Establish metrics and processes to monitor the effectiveness of our CHW interventions and enhanced care management activities. What We're Looking For The ideal candidate is a seasoned professional with significant experience in one or more of the following areas: Managed Care, Care Coordination, or Case Management (e.g., Nurse Case Manager, Social Worker, Health Plan Program Manager). Community Health Programs or Public Health. Program Management and Leadership within a non-profit or healthcare setting. Familiarity with the California healthcare landscape and models like Enhanced Care Management (ECM) and Community Supports (CS) is a strong plus. A passion for holistic health, health literacy, and equity . Exceptional organizational, communication, and team leadership skills.
