Job Description

We are looking for a Senior Python Backend Engineer to join Unosquare and work on high-impact, client-facing healthcare platforms for U.S.-based companies.

In this role, you will design and build interoperability-first backend services that securely move, transform, and expose clinical data, with a strong focus on Python, REST APIs, FHIR, and C-CDA, deployed on Azure-native, scalable architectures.

You will collaborate closely with product partners, architects, and engineers, contributing not only through hands‑on development but also through technical decisions, system design, and best practices, ensuring solutions are secure, scalable, and audit‑ready in HIPAA/HITRUST environments.

Responsibilities

  • Design, build, and maintain backend services and APIs using Python (FastAPI / Flask).
  • Develop and integrate FHIR-based services (Patient, DocumentReference, $match) with OAuth2 / SMART on FHIR authentication.
  • Parse and transform C-CDA clinical documents, mapping structured data for downstream systems.
  • Build and maintain HTML → PDF generation pipelines for compliant healthcare documentation.
  • Deploy and scale services using Azure Container Apps or Azure Function Apps (Elastic Premium).
  • Work with cloud data stores such as Azure SQL, Blob Storage, Data Lake, and Cosmos DB.
  • Implement logging, metrics, and tracing using Application Insights and Log Analytics.
  • Write clean, maintainable, and well‑tested code; participate in code reviews and CI/CD pipelines.
  • Contribute to architecture discussions, scalability strategies, and reliability improvements.
  • Collaborate effectively with distributed, cross‑functional teams in an agile environment.

Required Qualifications

  • 5+ years of professional experience as a Backend / Platform / Data Engineer.
  • Strong experience with Python, including FastAPI or Flask.
  • Proven experience designing and consuming REST APIs and OpenAPI/Swagger specifications.
  • Hands‑on experience with FHIR and C-CDA integrations.
  • Experience working with EMRs such as Epic, Cerner, Athena, or similar.
  • Solid experience deploying and operating scalable workloads on Azure.
  • Knowledge of event‑driven architectures and cloud data storage solutions.
  • Strong understanding of git workflows, CI/CD, and secure coding practices.
  • Excellent communication skills and ability to work with U.S.-based stakeholders.

Nice to Have

  • Experience with HL7 v2 (MDM / ADT) message handling.
  • Familiarity with HTML-to-PDF engines and templating systems.
  • Experience with Infrastructure as Code (Bicep, Terraform, ARM).
  • Azure API Management and private networking patterns.
  • Prior experience working in HIPAA/HITRUST-regulated environments.
  • Experience working directly with U.S. healthcare clients.
