Senior/Assistant Manager

Key Responsibilities:

1. Support the operations of the Community Care Team within the assigned area, including programmes such as Transition Care and Community Health Posts to help residents lead healthier lifestyles.
2. Engage and collaborate with community partners to coordinate health-related initiatives and strengthen community operations within the assigned zone.
3. Assist in operational planning, including analysing ops data, monitoring workload distribution, and tracking performance against operational KPIs.
4. Drive improvements in community operations by piloting new programmes and workflows, evaluating technology solutions, and assessing pilot outcomes for future enhancements.
5. Contribute to overall Patient Experience efforts by managing resident feedback, coordinating investigations and service recovery, and identifying service gaps with relevant stakeholders.
