The Service Coordinator will:
Write concise, accurate and objective Interdisciplinary Notes regarding the current status of the client and related service coordination activities on a monthly basis Communicats effectively in person, on the telephone and in writing Read, interpret, and follow complex policies, procedures, laws, and regulations Interact tactfully and effectively with those contacted in the course of work Maintain current workload with accurate and complete documentation Conduct meetings, interviews and evaluations; interpret and document data Visit clients in their homes, schools, and day programs Record accurately and timely on time sheet and mileage forms Perform other duties, as assigned Qualifications: 3+ years of experience in a field related to Developmental Disabilities with a Bachelor's Degree; or,...
