SIL Intake & Growth Coordinator

Responsibilities

• Actively manage vacancies across multiple SIL homes
• Build and maintain referral relationships with Allied health professionals, hospitals and discharge planners
• Follow up enquiries promptly and consistently
• Match participants to homes based on needs, compatibility, and support requirements
• Intake & admissions: Coordinate participant intakes and transitions into SIL homes; gather and review relevant documentation; work closely with the Service Coordinator to ensure safe and well-planned admissions
• Relationship management: Be a trusted and professional point of contact for referrers; represent the organisation authentically in the NDIS sector; keep referral partners informed of availability and suitability
• Reporting & visibility: Track enquiries, referrals, and admissions; provide clear updates on vacancy status and pipeline progress; identify trends, risks, and opportunities
