The newly renovated Avante at St. Cloud Skilled Nursing and Rehabilitation Center is seeking a Social Services Assistant to assist the Social Services department with providing medically related social services.
Essential Job Functions: Responsibilities include but are not limited to the following:
· Assist with discharge planning, including arrangements for home health services,
DME, transportation, circulation of post d/c plan of care, mail survey to
discharged residents, post discharge follow-up calls in the log, document steps
