**The Social Services Specialist responsibilities include:**
+ Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
+ The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
+ They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
+ Possess a clear understanding of discharge and transition planning and linkage with community resources
+ Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
**Position details:**
+ **Location:** St Josephs Hospital - Tampa, FL
+ **Status:** Part time, 16 hours per week
+ **Schedule:** 8:00 AM - 4:30 PM
+ **Weekend requirement:** Three weekends on and one weekend off
+ **On Call:** No
**Certifications and Licensures:**
+ Preferred ACM (Case Management) or
+ Preferred CCM (Case Manager)
**Education:**
+ Required Masters in Social Work, Sociology, or Related Field
+ Strongly Preferred Masters Degree in Social Work
**Experience:**
+ Required 6 months Internship or
+ Required 1 year Social Work in an acute care or hospital setting
