INFORMATION AND TRIAGE


• Manage enquiries and referrals.


• Conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.


• Refer clients to appropriate agencies for further intervention.


• Ensure proper documentation of referrals and intakes.


• Ensure reports are submitted in a timely manner.

CASEWORK AND COUNSELLING


• Co-manage cases (mainly low level of complexity).


• Conduct assessment, planning and intervention for cases


• Participate in networking and case management meetings.


• Ensure proper documentation of cases.


• Ensure reports on cases are submitted in a timely manner.

COMMUNITY WORK & GROUP WORK


• Network with other organizations and stakeholders.


• Participate in implementation and evaluation of community outreach / initiatives.
