Social Work Designee

St. Anthony Park Home is seeking a Social Work Designee to support residents and families in our skilled nursing facility.

Responsibilities

• Assist residents and families with psychosocial needs

• Support care planning, admissions, and discharge planning

• Coordinate with interdisciplinary team and outside resources

• Maintain required documentation and compliance

Requirements

• Social services or healthcare experience preferred

• SNF or long-term care experience a plus

• Strong communication and organizational skills

Schedule & Pay

• Full-time

• $22–$27/hr based on experience
