Job Description

Clinical Social Worker in the Collaborative Care is responsible to provide brief interventions using evidenced based techniques such as Behavioral Activation, Problem Solving treatment, Motivational Interviewing or other appropriate treatments in primary care settings. Work collaboratively with PCP and psychiatric consultant. Supports psychotropic medication management prescribed by PCP's focusing on treatment adherence, side effects and other complications and effectiveness of treatment. Participates in regularly scheduled caseload consultations with the Psychiatric Consultant and communicates resulting treatment recommendations to the patient's PCP, focusing on patient's new treatment or who are not improving as expected.

Responsible for documentation in the EMR and track patient's follow-up/clinical outcome in CMTS registry. Facilitate referrals for clinical indicated services outside of primary care clinic. Facilitate patient engagement...
