Social Worker - Nursing Home


Located near Deerfield, NJ

A skilled nursing, long term care facility located near Deerfield, NJ  is seeking a strong Social Worker. 

Responsibilities:

  • Plans and directs a comprehensive social services program which provides support services for residents and families.
  • Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
  • Participates in an interdisciplinary team approach to better serve the needs of the facility population.

    • Qualifications/ Requirements:

  • Must have a CSW or LSW within the State of NJ
  • Must have previous experience as a Social Worker
  • Must have nursing home experience
  • STC and LTC experience strongly preferred
  • Strong written and verbal communication skills
