The successful candidate will be well versed in team dynamics, multidisciplinary team rounding, collaboration, teaching and supporting families. The successful candidate will have previous experience in acute care pediatric tertiary hospital setting supporting new onset, maintenance and crisis interventions for chronic conditions. You will be a skilled communicator with well-established community resource supports and networks: schools, community agencies, and government programs. Your strengths-based approach and diversity and inclusion practices will assist the families to reach best outcomes for ongoing health success.
