Job Description
Responsibilities:
- Conduct and renew means tests and financial assessments for clients.
- Establish professional relationships with clients and caregivers to facilitate effective support.
- Perform initial screenings and assessments to understand the complexity and needs of cases.
- Develop, implement, and monitor care and case plans to align with clients' needs.
- Lead admission and discharge planning, ensuring active communication with families and the multidisciplinary team.
- Prepare written interdisciplinary care plans addressing psychosocial issues and intervention goals for each resident.
- Assist families in accessing resources, including financial assistance as needed.
- Coordinate and conduct Multi-Disciplinary Team (MDT) meetings, highlighting critical cases.
- Maintain timely and accurate documentation in the SW Communication Log Sheet and produce necessary reports.
