Job Description

Responsibilities:

  • Conduct and renew means tests and financial assessments for clients.
  • Establish professional relationships with clients and caregivers to facilitate effective support.
  • Perform initial screenings and assessments to understand the complexity and needs of cases.
  • Develop, implement, and monitor care and case plans to align with clients' needs.
  • Lead admission and discharge planning, ensuring active communication with families and the multidisciplinary team.
  • Prepare written interdisciplinary care plans addressing psychosocial issues and intervention goals for each resident.
  • Assist families in accessing resources, including financial assistance as needed.
  • Coordinate and conduct Multi-Disciplinary Team (MDT) meetings, highlighting critical cases.
  • Maintain timely and accurate documentation in the SW Communication Log Sheet and produce necessary reports.
