Job Description
This is a part time no benefits Social Worker position in an Article 31 OMH outpatient facility for one 8 hour shift a week. Located at 184 Court Street this is an active outpatient facility emphasizing a team approach to the best mental health care available.
Position Summary:
Supports psycho‐social needs of patients who are currently receiving or recently received hospital based care. The Social Worker strives to enhance the patient’s optimized level of health and/or recovery; provides interventions for populations ranging from infants to geriatrics; serves as a resource for the healthcare team, assists in identifying discharge planning needs; provides support / intervention and resources.
Education, License & Cert:
+ MNN ‐ Masters Degree Non Nursing: Masters of Social Work (MSW) required.
+ For staff providing care to patients in the state of New York, LMSW, LSW, LCSW is required
