Job Description

  • Evaluate and diagnose communication, cognitive-communication, voice, fluency, and swallowing disorders in patients across the lifespan.
  • Develop individualized treatment plans based on diagnostic findings and patient needs.
  • Provide therapy using evidence-based practices, including restorative and compensatory techniques.
  • Educate patients and caregivers on therapy goals, treatment strategies, home exercises, and safety considerations.
  • Document evaluations, progress notes, and discharge summaries accurately and timely within the electronic medical record (EMR).
  • Collaborate with physicians, nursing staff, rehabilitation team members, and dietitians to ensure coordinated patient care.
  • Participate in interdisciplinary team meetings and care conferences to review patient progress and update care plans.
  • Provide recommendations regarding communication support and swallowing precautions to enhance patient safety and outcomes.
