Job Description
Evaluate and diagnose communication, cognitive-communication, voice, fluency, and swallowing disorders in patients across the lifespan. Develop individualized treatment plans based on diagnostic findings and patient needs. Provide therapy using evidence-based practices, including restorative and compensatory techniques. Educate patients and caregivers on therapy goals, treatment strategies, home exercises, and safety considerations. Document evaluations, progress notes, and discharge summaries accurately and timely within the electronic medical record (EMR). Collaborate with physicians, nursing staff, rehabilitation team members, and dietitians to ensure coordinated patient care. Participate in interdisciplinary team meetings and care conferences to review patient progress and update care plans. Provide recommendations regarding communication support and swallowing precautions to enhance patient safety and outcomes.
