This is a Full-time position - 39 hours per week Responsibilities Develop and implement person-centred, effective care plans for residents and promptly communicate any changes in residents health status to the CNM/Assistant Care Manager. Work collaboratively with Health Care Assistants to prepare, develop, and deliver resident care plans. Maintain a safe and supportive environment for residents, staff, and visitors at all times. Collaborate with residents and multidisciplinary teams to ensure appropriate continuity of care. Deliver nursing care in line with evidence-based practice using a person-centred approach.
