An exciting opportunity is available for an experienced and professional Support Coordinator to support individuals in navigating and implementing NDIS funded support plans. This role focuses on working closely with participants and their chosen supports to build confidence, independence, and long-term capability in managing services. The position is suited to someone who is values-driven, proactive, and committed to achieving meaningful outcomes.
Key Duties and Responsibilities:
- Managing and progressing referrals for funded support coordination
- Working in partnership with participants and their families or representatives to strengthen their ability to manage supports
- Navigating funding frameworks and operating within established guidelines
- Responding effectively to changes or escalation in participant needs
- Building and maintaining strong professional relationships with a range of stakeholders
This is a permanent, full-time hybrid opportunity based in the West Perth region.
About You:
The ideal candidate will have prior experience in coordination, case management, or a similar role within the disability or human services sector, with a solid understanding of funded support systems. Formal qualifications are desirable but not essential; personal qualities and professional capability are equally valued.
