As our client base continues to grow, we are seeking an experienced Support Coordinator who shares our values and passion for person‑centred, holistic support.
About the Role
This role is ideal for someone who thrives in a flexible, self‑directed environment and is confident supporting participants with complex needs. You will manage an existing caseload, working closely with the Specialist Support Coordinator for guidance, supervision, and escalation support.
The position offers significant autonomy — you will manage your own calendar, structure your workday in a way that suits you, and balance working from home with community visits. The only non‑negotiable is that participant needs are met with consistency, professionalism, and care.
Key Responsibilities
- Provide high-quality Support Coordination in line with NDIS guidelines and best practice.
- Build strong, trusting relationships with participants, families, and service providers.
- Coordinate and monitor supports, ensuring services are aligned with participant goals.
- Navigate complex psychosocial, behavioural, and environmental factors with a trauma‑informed lens.
- Assist participants to understand and implement their NDIS plans.
- Maintain accurate, timely case notes and documentation.
- Manage your own schedule, appointments, and workload.
- Attend participant homes and community locations as required.
- Work collaboratively with the Specialist Support Coordinator for supervision, case review, and professional development.
About You
You will be a strong fit for this role if you bring:
- Experience working with mental health, AOD, neurodiversity, and/or neurological conditions.
- A solid understanding of the NDIS, particularly Support Coordination functions.
- Confidence working with participants who have complex needs or require a high level of advocacy.
- Excellent communication, problem‑solving, and organisational skills.
- The ability to work independently while staying connected to the team.
- A commitment to ethical, person‑centred service delivery.
Requirements
- Relevant qualification in community services, mental health, social work, disability, or similar.
- Current NDIS Worker Screening Check.
- Current Working With Children Check (or willingness to obtain).
- Driver’s licence and reliable vehicle.
- Experience in Support Coordination (highly desirable).
