Support Worker- Aged Care Worker

As a Community Support Worker (Aged Care Worker), you will provide respectful, personcentred inhome support to older people living in the community under the Support at Home program. Working primarily on a onetoone basis, you will deliver services autonomously in clients homes, following individual care plans and organisational policies. Your responsibilities may include: Assisting with personal care such as showering, dressing, grooming, toileting, mobility, and medication assistance (where trained) Providing domestic assistance including cleaning, laundry, meal preparation, and shopping Offering companionship and social support, encouraging participation in social and community activities Supporting transport and community access, including appointments and outings (where required) Observing, documenting, and reporting changes in clients wellbeing Maintaining accurate records in line with aged care standards and regulatory requirements Requirements To be successful in this role, you ...
