Support Worker

Position Overview
We are seeking an experienced Disability Support Worker to join our Perth Metro team. This role involves providing direct support to NDIS participants in line with individual care plans, promoting independence, community participation, and overall wellbeing.
Key Duties and Responsibilities
- Provide personal care support including bathing, toileting, grooming, dressing, mobility assistance, and hoisting for participants with varying levels of care needs.
- Assist participants with daily living activities and household tasks to support independent living.
- Support participants with behavioural and complex support needs in accordance with behaviour support plans.
- Encourage and facilitate community access, social engagement, appointments, and recreational activities.
- Implement and follow individual support plans and NDIS goals.
- Monitor participant wellbeing and report changes or concerns to relevant stakeholders.
- Maintain accurate case notes, progress reports, and documentation.
- Work collaboratively with families, carers, allied health professionals, and internal teams.
- Ensure compliance with NDIS Practice Standards, workplace health and safety requirements, and organisational policies.
Skills and Experience Required
- Minimum 1 year relevant experience supporting people with disability, including behavioural or high‑care needs.
- Certificate III or IV in Disability, Individual Support, or a closely related field.
- Sound knowledge of the NDIS framework and person‑centred support principles.
- Strong communication, empathy, and problem‑solving skills.
- Ability to work independently and as part of a multidisciplinary team.
Mandatory Requirements
- NDIS Worker Screening Check (or willingness to obtain).
- Completion of the NDIS Worker Orientation Module (Current).
- First Aid and CPR (or willingness to obtain).
- Manual Handling and Infection Control training (or willingness to complete).
- Current driver’s licence and reliable vehicle.
- Full‑time availability and flexibility to work varied shifts.
