About the Role
We are seeking experienced and compassionate care Support worker to provide high-level care for complex care client. This role involves delivering person-centered support, including assistance with daily living, personal care, behavior management, medication administration and clinical tasks as required.
Key Responsibilities
- Provide High-quality, person-centered care tailored to client's complex needs.
- Assist with personal care, mobility support and daily living activities.
- Support the client with social engagement and community participation.
- Follow individualized behavior support strategies.
- Work collaboratively with health care professionals and family members.
- Maintain Accurate documentation and progress notes.
Requirements
- Minimum (Cert III or IV) Disability, individual support, or equivalent.
- Minimum of 2 years of work experience.
