Position: Case Manager
Divison: Mental Health Housing Services
Program: Supportive Housing
General Overview or Main Functions: Oversight and management of the individual cases of all Supportive Housing tenants. Development of Tenant Individual Support Plans. Tenant linkage to Community Service Providers. Observation, documentation and reporting of tenant status.
Qualifications: Bachelors Degree in Social Work or related field preferred. Valid NYS Drivers License.
Hours: Full-Time.
Salary Range: $38,555 - $42,764, annually.
